For a new patient with Midwest Multicare, please bring the following items to your initial appointment.
- Valid Identification
- Insurance card, if applicable.
- Any previous testing or information to help us help you
- Smile and an open mind
Please plan on spending up to one hour for the first appointment. We want to be a thorough as possible before we make health decisions.
Please arrive ten minutes before your appointment to fill out any paperwork for Dr. Hunt.
